Physician Referral Instructions
Patients wishing to see a physician at Saskatoon Nephrology Group must be referred by a Family Physician, Nurse Practitioner, or Specialist Physician.
For all referrals, please fax the following items to our office at (306) 934-3355. Please note that we require the following information in order to accurately triage referrals:
- Physician referral name
- Physician referral phone/fax number
- Patient demographics
- Relevant history
- Renal abdominal ultrasound report (if already done)
- List of current medications
- List of co-morbidities
- Previous creatinine measurements in the past two years (if available) and any available urine test results
CANADA POST STRIKE
Due to the ongoing Canada Post Strike, we are unable to send appointment information by mail. We have two options available, appointments can be sent by email or by joining the Medeo app. Please call the office to make sure we have your email and/or get you signed up for the Medeo app asap!
