Physician Referral Instructions

Patients wishing to see a physician at Saskatoon Nephrology Group must be referred by a Family Physician, Nurse Practitioner, or Specialist Physician. 

For all referrals, please fax the following items to our office at (306) 934-3355. Please note that we require the following information in order to accurately triage referrals:

• Physician referral name
• Physician referral phone/fax number
• Patient demographics
• Relevant history
• Renal abdominal ultrasound report (if already done)
• List of current medications
• List of co-morbidities
• Previous creatinine measurements in the past two years (if available) and any available urine test results
